Pharmacological properties
Pharmacotherapeutic group:
Agent reducing gastric gland secretion; proton pump inhibitor. ATC code: A02BC04.
Pharmacodynamics:
Rabeprazole belongs to the class of substituted benzimidazoles. It suppresses hydrochloric acid secretion by inhibiting H+/K+-ATPase (the proton pump) of gastric parietal cells; the antisecretory effect develops within one hour after administration.
Pharmacokinetics:
With intravenous administration, bioavailability is 100%. Plasma protein binding is about 97%. Metabolized in the liver with the participation of CYP2C19 and CYP3A4 isoenzymes; the half-life is 1–2 hours; excreted mainly by the kidneys as metabolites.
Contraindications
Hypersensitivity to rabeprazole, substituted benzimidazoles or components of the drug.
Children under 18 years of age.
With caution: severe renal and hepatic impairment.
During pregnancy, use only if the expected benefit to the mother outweighs the potential risk to the fetus; if use is necessary, breastfeeding should be discontinued.
How to use
Administered intravenously when oral intake of proton pump inhibitors is impossible. The recommended dose for adults is 20 mg once daily.
Intravenous injection:
Dissolve the contents of the vial in 5 ml of water for injection and administer slowly, over 5–15 minutes.
Intravenous infusion:
Dilute the prepared solution in 100 ml of 0.9% sodium chloride solution and administer over 15–30 minutes.
Use only water for injection and 0.9% sodium chloride solution for dissolution and dilution.
Composition
Each vial contains:
- Rabeprazole sodium (as lyophilisate) — 20 mg
Side effects
Digestive system:
Common — diarrhea, nausea, vomiting, abdominal pain, flatulence, constipation; rare — dry mouth, belching, taste disturbance.
Nervous system:
Common — headache, dizziness, insomnia; rare — drowsiness, nervousness.
Allergic reactions:
Rare — rash, skin itching, erythema; in isolated cases — severe hypersensitivity reactions.
Other:
Asthenia, flu-like syndrome, back pain and peripheral edema are possible.
